ABSTRACT

In order to optimize the clinical service experience and provide efficient, convenient and high-quality logistics services, a multi-campus hospital has taken a series of measures, including establishing a logistics one-stop service center, building a logistics operation and maintenance management platform based on mobile Internet, normalizing quality control circle activities, building logistics command and dispatch center and business data center, and establishing performance evaluation system of logistics service personnel, etc., to provide homogeneous logistics service. The practice of multi-campus logistics management based on one-stop service mode can improve the level of logistics service, improve the clinical experience of logistics service, improve the satisfaction and sense of belonging of logistics staff, and play an important role in improving the rapid response ability of the hospital.

Objective@#To analyze the effect of ABC classification method on improving the drug management system of Urumqi Friendship Hospital.@*Methods@#The drug data of Urumqi Friendship Hospital from January 2017 to December 2017 were retrospectively analyzed.The in-hospital information system (HIS) was used to sort the annual consumption, and the antibacterial drugs were sorted by DDDS to analyze the data as control group.From January 2018 to December 2018, the drug data of Urumqi Friendship Hospital were retrospectively analyzed by the AIS analysis method in the hospital HIS system, which were selected as research group.The effect of ABC analysis on the management of hospital drugs was analyzed, and to compare the changes in drug management before and after the application of ABC analysis.@*Results@#Through the analysis of A, B and C drugs, the total amount of 921 drugs were found to be 127.358 million CNY, including 122 kinds of A drugs (13.25%), the amount of 97.975 million CNY (79.93%); 133 kinds of B drugs (14.44%), the amount of 18.56 million CNY (15.17%); 666 kinds of C drugs (72.31%), the amount of 6.0016 million CNY (4.90%). Among the ABC classification results, there were 24 kinds of cardiovascular system drugs in class A drugs, and the highest percentage was 28.57%.There were 29 kinds of Chinese herbal medicines in class B drugs, with a percentage of 23.33%.There were 52 kinds of oral drugs (42.62%), 64 kinds of injections (52.46%), and 6 kinds of other drugs (4.92%), while 66 drugs (49.62%) were classified as B drugs.Species (44.36%) and 8 other drugs (6.02%). Among the ABC classification results, the total amount of the top 20 drugs in class A drugs was 46 073 529.65 CNY, accounting for 47.11% of the A drugs, accounting for 37.65% of the total amount.Among them, the amount of clopidogrel tablets and atorvastatin calcium tablets was significantly larger.The inventory turnover index [(8.37±2.48)d] of the inventory management index of the research group after the ABC classification method was significantly higher than that of the control group [(6.22±3.61)d] (t=14.897, P<0.05), and the monthly average inventory amount [(4.5 354±0.5 024)million CNY] was significantly higher than that in the control group [(3.9 963±0.7 538)million CNY] (t=18.060, P<0.05), the slow-moving rate [(4.24±1.31)%] and the stock-out rate [(8.73±3.14)%] were significantly lower than those in the control group [(5.11±0.82)%, (9.39±2.67)%] (t=17.084, 4.859, all P<0.05).@*Conclusion@#The ABC classification method helps to improve the hospital drug management system, can promote rational drug use, dynamically manage drugs, ultimately reduce drug costs, and rationally utilize limited drug resources.

The inventory turnover index [(8.37 ±2.48)d] of the inventory management index of the research group after the ABC classification method was significantly higher than that of the control group [(6.22 ±3.61) d] ( t=14.897, P<0.05),and the monthly average inventory amount [(4.5 354 ±0.5 024)million CNY] was significantly higher than that in the control group [(3.9 963 ±0.7 538) million CNY] (t=18.060,P<0.05),the slow-moving rate [(4.24 ±1.31)%] and the stock-out rate [(8.73 ±3.14)%] were significantly lower than those in the control group [(5.11 ±0.82)%,(9.39 ±2.67)%] (t=17.084,4.859,all P<0.05).Conclusion The ABC classifica-tion method helps to improve the hospital drug management system ,can promote rational drug use ,dynamically manage drugs,ultimately reduce drug costs ,and rationally utilize limited drug resources.

ABSTRACT

O presente trabalho busca detalhar uma experiência pioneira de desenvolvimento de um sistema de informação em saúde voltado para subsidiar a gestão com base em evidências junto à atenção primária à saúde (APS). O processo de estruturação de programas e políticas públicas muitas vezes se dá sem acesso às melhores evidências científicas. Nesse contexto, o Sistema Integrado de Informações Mais Médicos (SIMM), aqui descrito, materializou um esforço para suprir a lacuna de informações para a gestão em saúde. Criado com objetivo de integrar os dados das principais fontes de informação relativas ao Projeto de Cooperação entre governo brasileiro e Organização Pan-Americana da Saúde/Organização Mundial da Saúde (OPAS/ OMS) para provimento de médicos no Programa Mais Médicos (PMM), o SIMM buscou otimizar a geração e a consolidação de informações que possam subsidiar decisões, além de apoiar o controle administrativo e financeiro. A experiência ilustra o desenvolvimento de uma solução auxiliar que pode ser ampliada para monitorar parâmetros de recursos humanos em saúde não apenas no PMM, mas na APS como um todo. A expertise desenvolvida criou condições para que o sistema pudesse ser configurado como um bem público, para além do suporte à gestão de um projeto em específico. O sistema pode ser adaptado e replicado em outros países das Américas para melhorar a disponibilidade e a qualidade das informações utilizadas por tomadores de decisão.(AU)


The present work provides details about a novel experience involving the development of a health information system focused on supporting evidence-based management at the primary health care (PHC) level. The process of structuring public health programs and policies is often developed without access to the best available scientific evidence. In this context, the proposed system (Sistema Integrado de Informações Mais Médicos, SIMM) materializes the effort to bridge the information gap for health management. Created with the aim of integrating data from the main information sources associated with the Cooperation Project between the Brazilian federal government and the Pan American Health Organization/World Health Organization (PAHO/ WHO) for provision of physicians for the More Doctors Program, SIMM strove to optimize the generation and consolidation of information to support decision-making, in addition to providing support for administrative and financial control. The experience illustrates the development of an ancillary solution that can be expanded to monitor human resources for health parameters across the entire PHC setting, far beyond the More Doctors Program. The resulting expertise created conditions for the system to be configured as a public asset, rather than being restricted to providing managerial support for a specific project. SIMM may be adapted and replicated in other American countries to improve the availability and quality of the information used by decision makers.(AU)


En el presente trabajo se detalla una experiencia pionera de desarrollo de un sistema de información de salud centrado en apoyar la gestión basada en la evidencia en el nivel de la atención primaria de salud. Muchas veces, el proceso de estructuración de los programas y políticas públicas se realiza sin acceso a las mejor evidencia científica. En ese contexto, el sistema integrado de información de Mais Médicos (SIMM) que se describe aquí materializó un esfuerzo para suplir la carencia de información para la gestión en materia de salud. El SIMM, creado con el objetivo de integrar los datos de las principales fuentes de información relativas al proyecto de cooperación entre el Gobierno del Brasil y la Organización Panamericana de la Salud/Organización Mundial de la Salud (OPS/OMS) para la provisión de médicos destinados al programa Más Médicos, se utilizó para optimizar la generación y consolidación de información a fin de sustentar las decisiones, además de servir de apoyo para el control administrativo y financiero. La experiencia ilustra el desarrollo de una solución complementaria que puede ampliarse para observar los parámetros de recursos humanos en salud no solo en el programa Más Médicos, sino en todo el ámbito de la atención primaria de salud. La pericia adquirida creó condiciones propicias para poder configurar el sistema como un bien público, con miras a ampliar su alcance más allá del apoyo a la gestión de un proyecto específico. Es posible adaptar y reproducir el sistema en otros países de la Región de las Américas para mejorar la disponibilidad y la calidad de la información utilizada por los responsables de tomar decisiones.(AU)

ABSTRACT

The authors attempted to use information technology in hierarchical management on clinician′s surgical authority. By means of a hierarchical surgery catalogue database, clinicians′ surgical authority is subject to by-level IT-based approval, and such authorities as clinician′s surgical medical advice, application for surgery, and approval of special surgeries are regulated. Thanks to multi-dimensional objective data, clinicians′surgical competence is subject to a dynamic evaluation, hierarchical authorization and reauthorization. These measures further standardize the behavior of the surgeons, and rule out unauthorized operations, thus improving fine management of surgeries and ensuring patient safety.

ABSTRACT

ABSTRACT Objective: To verify the use of procedures record tools as data source for monitoring and assessment of Psychosocial Care Centers (CAPS- Centros de Atenção Psicossocial). Method: A descriptive, exploratory qualitative study was carried out in seven CAPS in the state of Goiás. A total of 58 professionals participated, and the data collection was from April to May 2016 through focus groups. The data were submitted to the thematic analysis of content with the aid of the ATLAS.ti 6.2 software. Results: Three thematic categories emerged from the content analysis: Understanding about the record of Psychosocial Care Centers procedures; Management of the record tools of Psychosocial Care Centers procedures; and Intervening factors for recording Psychosocial Care Centers procedures. Final considerations: Investment in the Permanent Education of professionals will be necessary; focusing on the management of the records resulted from the actions developed in the CAPS to qualify the information and the work process of the professionals.


RESUMEN Objetivo: Verificar el uso de los instrumentos de registro de procedimientos como fuente de datos para monitoreo y evaluación de Centros de Atención Psicosocial (CAPS). Método: La investigación descriptiva, exploratoria de naturaleza cualitativa, realizada en siete CAPS del estado de Goiás. Participaron 58 profesionales, y la recolección de datos fue en el período de abril a mayo de 2016 por medio de grupos focales. Los datos se sometieron al análisis temático de contenido con la ayuda del software ATLAS.ti 6.2. Resultados: En el análisis de contenido se incluyeron tres categorías temáticas: Comprensión sobre el registro de procedimientos de CAPS; Manejo de los instrumentos de registros de procedimientos de CAPS; y Factores intervinientes para registrar los procedimientos de CAPS. Consideraciones finales: Será necesario la inversión en la Educación Permanente de los profesionales, con foco en el manejo de los registros generados a partir de las acciones desarrolladas en los CAPS para calificación de las informaciones y del proceso de trabajo de los profesionales.

ABSTRACT

In order to improve nursing informationization,and to promote the development of nursing informatics, our nursing department set up a new position called informatics nurse and a series of working systems and man-agement methods were developed. After implementation,the informatics nurse had achieved various goals,including:playing a role in developing 60 nursing information system programs,improving 11 medical and nursing process modification,collecting 399 items of nursing information system problems with 338 items solved,completing 22 nurs-ing information system using instructions,and organizing training meetings and lectures in the hospital for 12 times. The informatics nurse has been appreciated by medical staff and IT engineers who was believed to have promoted the development of nursing informatics in our hospital.

ABSTRACT

As it becomes normal for the PLA navy to sail to the open sea for training and the the multiple kinds of offshore military tasks it will face on the sea, the navy officers and soldiers will face more complicated health care problems, which brings greater challenges to the naval medical service support. In this review, we started from the problems in the medical service support; using the spatial analysis functions of geographic information' system (GIS), and designed a navalmedical service support GIS functional model, which included three functional modules: monitoring of the woundedand diseased, informationmanagement of naval medical service resource, and assistant decision-making for military medical service; and we also discussed the application of GIS in monitoring and early warning of the diseases, analysis and evaluation of risk factors, health resource allocation and decision-making support for health service.

With the extensive application of informationalized management systems for barcode specimens , the degree of informatization is becoming higher and higher in medical laboratory.Informationalized management combines modern information technology and advanced management concepts , transforms or reengineers the laboratory operation and business process.Test specimen turnaround time ( TAT) is an important factor affecting the quality of the inspection.By analyzing the test process of each time node , establish the suitable specimens monitoring program for clinical requirements and real -time monitor the key nodes in test processes ,which will effectively shorten TAT , improve reporting timeliness rate and avoid clinical complaints.

ABSTRACT

OBJECTIVES: The purpose of this study was to review evaluation studies of nursing management information systems (NMISs) and their outcome measures to examine system effectiveness. METHODS: For the systematic review, a literature search of the PubMed, CINAHL, Embase, and Cochrane Library databases was conducted to retrieve original articles published between 1970 and 2014. Medical Subject Headings (MeSH) terms included informatics, medical informatics, nursing informatics, medical informatics application, and management information systems for information systems and evaluation studies and nursing evaluation research for evaluation research. Additionally, manag* and admin*, and nurs* were combined. Title, abstract, and full-text reviews were completed by two reviewers. And then, year, author, type of management system, study purpose, study design, data source, system users, study subjects, and outcomes were extracted from the selected articles. The quality and risk of bias of the studies that were finally selected were assessed with the Risk of Bias Assessment Tool for Non-randomized Studies (RoBANS) criteria. RESULTS: Out of the 2,257 retrieved articles, a total of six articles were selected. These included two scheduling programs, two nursing cost-related programs, and two patient care management programs. For the outcome measurements, usefulness, time saving, satisfaction, cost, attitude, usability, data quality/completeness/accuracy, and personnel work patterns were included. User satisfaction, time saving, and usefulness mostly showed positive findings. CONCLUSIONS: The study results suggest that NMISs were effective in time saving and useful in nursing care. Because there was a lack of quality in the reviewed studies, well-designed research, such as randomized controlled trials, should be conducted to more objectively evaluate the effectiveness of NMISs.

ABSTRACT

Introducción: se exponen en el trabajo los primeros resultados obtenidos en el desarrollo del sistema de telemonitorización de pacientes críticos, ejecutado como parte de un proyecto ramal de la Dirección Nacional de Urgencias Médicas del Ministerio de Salud Pública de Cuba. El sistema se desarrolla a partir de los requerimientos de diseño definidos por el grupo de emergencistas e intensivistas de todo el país que participaron de la investigación. Objetivo: establecer una estructura única al registro de los procedimientos que se desarrollan en la atención al paciente, mejorando con ellos la recepción y el procesamiento de los datos que se registran, para la posterior emisión de resultado. Material y método: el sistema es una aplicación web con arquitectura cliente-servidor, desarrollada con el framework JBoss Seam en lenguaje Java. Se ha desarrollado de forma modular, con una interfaz para la navegación de los médicos y enfermeros muy sencilla e interactiva. Resultados: el sistema se ocupa de la gestión y el registro de las actividades clínicas y administrativas que se llevan a cabo en un servicio de hospitalización, mediante el sistema de información clínica, así como de la comunicación con los equipos médicos que monitorizan al paciente en su cabecera. Conclusiones: se facilita la captura, gestión, tratamiento y almacenamiento de la información generada para cada paciente hospitalizado, integrando toda la información que se maneja en el servicio. Se han desarrollado los módulos correspondientes al registro del recibimiento médico, la indicación médica de tratamiento y su cumplimiento por parte de enfermería, la prescripción de medicamentos al paciente, las gestiones de medicamentos en la farmacia y de las dietas con el departamento dietético, entre otros.


Introduction: the first results in the development of telemonitoring system for critically ill-patients carried out as a part of a project of the National Medical Emergencies Direction, Cuban Public Health System is presented in this research paper. The system is developed from the requirements of a design that was defined by the group of medical emergency and critical care personnel all over the country participating in this research. Objective: to establish a unique structure to register the procedures that are performed in the patients care, improving by means of them the reception and data processing storage to the posterior transmission of results. Material and method: the system is a web application having user-server architecture, developed with JBoss Seam framework in Java language. It has been developed of modular structure with an interface to the navigation of medical doctors and nurses; which is very simple and interactive. Results: the system deals with the management and registration of the clinical and administrative activities carry out in a service of hospitalization through the system of clinical information, as well as the communication with the medical teams monitoring at the patient's bedside. Conclusions: capture, management, treatment and storage of information generated for every hospitalized patient in the service is facilitated. The corresponding modules related to medical admission, indication of treatment and fulfillment by the nursing personnel, along with the prescription of medications to patients, managing of medications in the pharmacy and diets with the dietetics department among other, have also been developed.

Resumen:


Introducción: el estudio de los sistemas de información se produjo como una sub-disciplina de las ciencias de la computación en un intento por entender y racionalizar la administración de la tecnología dentro de las organizaciones, sin embargo, constituye una herramienta tecnológica útil en la toma de decisiones.


Desarrollo: el Sistema de Información Administrativa de los Recursos Humanos de Enfermería (SIARHE) es uno de los proyectos más ambiciosos de la Comisión Permanente de Enfermería, mediante el cual se pretende contar con información confiable, oportuna y completa para detectar y atender con oportunidad las necesidades de recursos de enfermería y su mejor distribución a nivel nacional e institucional y en consecuencia, lograr el equilibrio entre la oferta y la demanda de estos recursos. Conclusiones: la información reportada por el SIARHE, está enfocada a lograr la caracterización del personal de enfermería desde el punto de vista académico, laboral y profesional para coadyuvar en la formulación de políticas que permitan conformar la fuerza de trabajo de enfermería suficiente y calificada, para responder de manera efectiva a las demandas de salud del país. Los avances al momento son significativos gracias a la contribución de las instituciones que conforman el sistema nacional de salud, en la consolidación del SIARHE, recurso administrativo necesario para asegurar la continuidad en la operación del sistema.


Abstract:


Introduction: the study of information systems occurred as a sub-discipline of computer science in an attempt to understand and rationalize the administration of technology within organizations; however, it is a useful technological tool in decision making.


Development: management Information System of Nursing Human Resources (Abbreviated in Spanish as SIARHE) is one of the most ambitious projects of the Standing Nursing Committee, through which it aims to have reliable, timely and complete information to detect and respond to the needs of nursing resources and for a better distribution to national and institutional level, thus achieving balance between supply and demand for these resources.


Conclusions: the information reported by the SIARHE, is focused on achieving nursing characterization from the academic, professional and work-setting standpoint to assist in the formulation of policies that will shape the skilled and enough nursing workforce to effectively respond to the health needs of the country. Progress to date are significant due to the contribution of the institutions of the national health system in the consolidation of SIARHE, administrative device necessary to ensure the continued operation of the system.

Clinical laboratory information system is the key to the realization of total laboratory automation,standardization,intelligent and digitization.In recent years,with the establishment,application,popularization and upgrading of clinical laboratory information system,it is very important to establish the construction criterion of clinical laboratory as soon as possible.Now according to the requirements of construction and management of modern clinical laboratory and combining with the practical application of clinical laboratory information system in our hospital at domestic settings,the standardization construction of clinical laboratory information system was discussed in this article.

ABSTRACT

Este artigo apresenta o estudo da inclusão do vetor externo, Acordo de Resultados, influenciou no Modelo de Gestão da FHEMIG. O ACORDO DE RESULTADOS viabilizou o monitoramento quantitativo, gestão de indicadores assistenciais e ações estratégicas conduzidas Fundação Hospitalar do Estado de Minas Gerais (FHEMIG), alinhados externamente às metas governamentais, e internamente aos diversos setores das Unidades Assistenciais. Este estudo pretendeu verificar se e como o Acordo de Resultados catalisou o processo de gestão na Rede FHEMIG sumarizado no Modelo de Gestão FHEMIG. O Modelo de Gestão FHEMIG apresenta-se como uma proposta de gestão integrada por três vetores tecnológicos, já validado e testado, aplicável a outras instituições no setor público, principalmente hospitais. O Modelo apoia-se nos seguintes vetores principais: (1) GESTÃO HOSPITALAR INTEGRADA, que evidencia ações tático/gerenciais/estratégicas, apoiadas em eixos de gestão centrada na gestão de custos hospitalares; (2) SOFTWARE INTEGRADO DE GESTÃO, que dá sustentação informatizada, inovadora, integrada às áreas assistenciais, utilizando dois métodos ­ Custeio por Absorção e Custeio por Atividades. Isto, aliada à proposta também inovadora de uso dos custos como meta informação custo. Por fim, (3) LABORATÓRIO DE GESTÃO, onde a FHEMIG se torna ambiente para estudos empíricos e para obtenção de informação. A aplicação do novo vetor ­ ACORDO DE RESULTADOS ­ visa compreender resultados quantitativos que representem evidências para a gestão do Governo de Minas e do SUS. O estudo, metodologicamente, constituiu-se num estudo de caso descritivo. Consiste em uma pesquisa aplicada utilizando a meta-informação custo.(AU)


This paper presents the study of how the inclusion of an external vector, the Results Agreement, internally influenced in FHEMIG's Management Model. The RESULTS AGREEMENT enabled the quantitative monitoring and management of care indicators and strategic actions conducted at the Hospital Foundation of the State of Minas Gerais (FHEMIG), lined, externally, to the government targets, and internally to the several sectors of Assistance Units, influencing the adoption FHEMIG's Management Model. This study sought to determine whether and how the agreement catalyzed the results management process in FHEMIG Network. The FHEMIG's Management Model presents itself as an integrated proposal for three new technologic vectors, already tested and validated, applicable to other institutions in the public sector, especially hospitals. The model relies on the following main vectors: (1) INTEGRATED HOSPITAL MANAGEMENT, which highlights tactical / managerial and strategic actions, supported by management axes that enabled the implementation aprimoraof the management system focused on managing hospital costs in line with other internal projects such as Hospital Accreditation and Clinical Protocols. (2) MANAGEMENT INTEGRATED SOFTWARE, which gives computadorized support to the system FHEMIG (laboratories, CMEs, ICUs, pharmacies, laundries, etc.), an innovative proposal for integrated work of two methods in a unified way ­ Costing by Absorption and Costing Activities. This, aligned to the also innovative proposal of use of information costs as a target cost. Finally, (3) MANAGEMENT LABORATORY. FHEMIG becomes environment for empirical studies and to obtain results, including the generation of Accountability to the patient, intended to be a source of information and transparency of public spending and clarification to the population served. The application of new vector ­ RESULTS AGREEMENT ­ aims to understand the present quantitative results that represent evidences for the SUS management and service providers basing negotiations on funding of public health and for transparency, visibility, public sector spending. The study, methodologically, consisted of a descriptive case study. It consists of an applied research using meta-information cost.(AU)

ABSTRACT

Health research oriented to solve the most relevant sanitary problems in Chile must be encouraged. In 2001, the National Health Research Fund (FONIS) was created by the National Research Council of the Ministry of Health and the National Scientifc Research Commission, to stimulate relevant health research that contributes to develop health care policies. In 2008 an experts meeting proposed eighty research areas. These areas were grouped in twelve thematic containers. Each of these containers were classifed as having maximal, intermediate or minimal priority. The seven most important containers were grouped in three areas. Among the latter, two were selected. One is evaluation of the Ministry programs and, within this area, with the following priorities in decreasing importance: primary prevention, health care priorities, and diseases included in the Explicit Guarantees plan. The second area corresponds to diseases with high prevalence, incidence, costs or impact, including the following priorities in diminishing importance: mental health, diseases of high prevalence and problems with social impact.

ABSTRACT

With the institutional and market changes in hospitals, strategic management has become increasingly important. Strategic managerial innovations can have impact on market pioneering and hospital performance, enabling better customer satisfaction, productivity improvement, and quality management. Hospital Information systems also require strategic planning to improve functionality in all aspects. This article reviewed the concept of strategic management and strategic information system (SIS), and summarized a framework for developing SIS for hospitals. The SIS formulation process consists of two interrelated phases: organizational analysis and SIS development. The first phase involves an analysis of the hospital's current position and the development of the hospital's vision and goals. The second phase incorporates three activities: SIS goals assessment, strategic information requirements analysis (SIRA), and SIS implementation planning. In particular, in SIRA, four dimensions of SIS and their interrelationships between dimensions would permit an organization to specify the strategic information needs within a hospital. Hospitals that integrate this framework with other aspects of their own hospital management information systems cope more effectively with the rapid changes and challenges encountered in today's health service industry.
